The screws on the gated ventral attachment point of the harnesses can prematurely wear and cause the ventral D-ring’s pin to release and attachment point to open.
The gate can malfunction and may not close automatically on affected carabiners when used.
The spring mechanism may break and the carabiner’s gate will not return to its closed position automatically when used.
The auto belay device can fail to retract when used.
When these chocks are used in a seaside or humid environment with high concentrations of salt in the air corrosion may occur and the chock may fail.
The recalled transceivers may not switch from SEND mode into SEARCH mode.
The rope may get stuck in the product's blocking slot and the user may not be able to remove the rope from its position.
A 10mm bolt will not fit into the 10mm bolt slot.
In some instances, the screwgate (the black part) in the carabiner may be assembled upside down.
Affected products have a very high and unusual tendency towards oxidation in marine environments.
The nylon runner may not be correctly sewn through. This is identified by the presence a tape splice on the runner.
The carabiner gates may not have been correctly pinned & the screwgate closure barrels may be incorrectly installed or the stopper rings may not be correctly positioned.
In some Index Ascenders, the rivet attaching the toothed cam to the ascender body may not be properly finished.
Some units may have been improperly assembled and may fail during use. The incorrect assembly refers to the rivets on the cam lobes not being properly pressed.
In some Via Ferrata lanyards, the security stitching (three orange bar tacks that attach the carabiners to the lanyards) may not be present.
Interaction between the internal coil spring and the gate pusher may cause the gate pusher to displace, potentially preventing the gate from closing or the mechanism from locking completely.
Following extensive testing, the supplier has found some units that did not achieve their rated strength. The rated strength of the devices varies from 4kN to 12kN.
Karabiners, from certain batches, may not hook up as effectively as they were designed to do so.
There are certain circumstances, particularly on tensioned ropes, where the device does not lock onto the rope as it should and may slip when initially loaded.
