Due to the tight fit of the plastic cap, small, threadlike (up to approximately 1cm long x less than 1mm wide), soft slivers of white plastic could peel off the jar cap upon opening, potentially contaminating the product.

Airbag inflator: As it gets older, a combination of high temperatures and humidity can cause the airbag inflator propellant to degrade. If an affected vehicle is involved in a collision triggering the airbag, the metal inflator housing may explode/rupture under too much internal pressure.

Product is potentially dangerous if used without restraining the infant with the safety harness. Infants may roll from their backs to their stomach or side while unrestrained. Over 30 infant fatalities have occurred overseas, since 2009, in Rock ‘n Play Sleepers, after the infants rolled over while unrestrained, or under other circumstances. Around 4,000 units were supplied in Australia between 1 Jan 2010 and 31 Dec 2013.
